Treatment Notes Blue background - Unable to Document

Has anyone experienced the treatment Notes as a blue screen (Screen shot attached)?

If so do you know the cause/fix.

I have 1 provider who is experiencing this issue and he can’t document any treatment notes.  It started recently.  He didn’t upgrade his plugin recently.  I’m not aware of anything

Here are the things I’ve done to recreate the issue.
I confirmed we have the same Chrome version installed.
I Installed the same Plug in Version
Assigned myself to the same security role
Copied their My Settings
Copied their Plug In Settings

Tomorrow I’m going to try and use the same computer that they are using and play with the resolution

If I’ve overlooked something that you can think of to try please let me know

I already have a ticket open. eCW is using the usual we don’t know root cause so we’ll throw spaghetti at the wall and hope something works. (i.e. clear cache, upgrade the plugin; however, the release notes don’t mention this issue).

I’m still going through with what they ask me to do; however, it is a provider who is in another city so responses from him isn’t the speediest and I’d prefer to find the root of the issue like to be able to recreate the issue.  I’m not aware of anything that was installed recently on his computer that would have started this behavior.
