We are trying to shorten up the time it takes for patients to fill out our new patient forms. We looked into transitioning to the portal but didn’t really care for the way it looked. We have been gathering forms from other resources to see how others look. We are a multi specialist practice that is owned by a hospital. We initially tried to use one form for all practices but are thinking about keeping the same demographic form but then having separate medical history forms by specialist. How are others doing this? Anyone care to share what their forms look like?
