Disappearing surgical history - this is a significant PATIENT CARE PROBLEM and providers should check it out

I’ve been investigating a significant patient care problem that many(most) providers aren’t even aware of.  It has been an issue for at least several years and possibly longer.  I found 1 similar thread on user forum from 2012 but it is in the product enhancement folder.  That thread referred to version 9 but I have been on version 10 for years with this problem.

In certain situations, surgical history that has been entered into the system will disappear.  I have been monitoring this and it occurs for me about 2-5 times PER WEEK or sometimes more.

Here is the scenario:
1) Patient has surgery.
2) Operative report shows up in my documents for review.  I enter the surgery through the right chart panel and receive the pop up box that says “changes made to the surgical history will not be reflected in encounters occurring prior to the current date and time.”  (Of course that would be the case and this is a wasted box but that isn’t the real problem).
3) The next time the patient comes for an office visit and the surgical history is opened (using any of the 3 ways to open the history), the surgery disappears from the list.  This happens if the patient has already checked in but not if you are preloading before the patient arrives.
4) The only way to preserve the surgical history once the patient has arrived is to open the History tab of the right chart panel then click the blue import arrow above the Surgical History.

This is predictable enough that I have screen shots (see attachment for one of many that I have - note the bladder tumor removal that disappeared) and I have even taken videos with my phone.  I showed a video to the support group at the National conference in Texas and it attracted alot attention but the problem continues. 

If you enter surgeries through the right chart panel,you should check it out to see if it is happening to you.
