Red Exclamation appearing on Allergies after patient intake

I’ve rooted out the causes of most of the red exclamation points on the progress notes indicating that changes will not transfer to right panel. Causes include Templates or clicking “Verified” on a section. Aside from the fact that neither of those should cause this behavior, I am still routinely seeing a red exclamation point on the Allergies section after my assistant performs the intake. She is not checking “Verified” on any sections, just copies the HPI questionnaire we use from the previous visit, then adding updates to sections. This warning means that if I go in and correct an allergy from non-structured to structured, or add a new allergy that was discovered during the appointment, these crucial changes are not transferring to further appointments. The work around is to edit the current progress note’s Allergies section and THEN edit the Hub’s Allergies section.

Anyone else seeing this just on Allergies? It doesn’t happen in every instance, but the majority. The only thing I’m seeing on the access logs, after I’m done banging my head on the keyboard when I notice the entries aren’t listed chronologically, is that the user “viewed” and then “created” the Allergies section, but there was no entered information.
