Jan 1 2020 Medicare requires CDS for all imaging orders, really?

Does anyone know the details and how it will impact ordering any CT or MRI on a Medicare patient?
Apparently providers will have to pay for a licensed clinical decision support tool “consultation” in order for Nedicare to cover imaging.  And if I’m correct,  Medicare’s site does not list ECW has having a certified clinical decision support tool that qualifies.


Pathology Documentation

We have a dermatologist in our practice that is also a pathologist.  Does anyone have a method for an internal pathologist to document their full pathology interpretation?  She is currently use a progress note but this does not display like a typical external pathology report.  I’m trying to find info on the personalized lock style just in case this is an option.

API Activation for MIPS

Has anybody activated the FHIR API, as required to meet the MIPS PI “Provide Patient Access” measure? Most of our practices have not yet activated it due to HIPAA privacy concerns. I’m just wondering if anybody has any experience with using this and if any security concerns have been encountered.

Releasing the Concurrency Lock

Is anyone having issues with the Concurrency Lock?  We have so many v11e issues, I seriously cannot tell if its eCW or us! Everyday I get complaints from users that sections of the charts are locked.  When I go in to Release Concurrency Lock, there are locked sections from days to weeks old!  We have all of our settings correct, as far as I can tell and have set our time limit down to 5 minutes to try to avoid this.  Any eCW technician I talk to lately, seems a bit clueless about these features.
Any insight is appreciated.

eCW Browser Slowness - JavaScript JS Heap Size

This is only applicable for eCW browser…

After upgrading to Version 11 in October, I have received a lot of complaints from our front-office staff that eCW becomes sluggish some time after logging on and using it for a while.

As a part of troubleshooting the sluggish performance, one of our IT techs asked me to take note of the JavaScript heap size found in Chrome Developer Tools > Memory > JS Heap Size when a user complains of slowness. What I have found is that the heap size is way over 100mb (sometimes close to or even over 200mb) whenever the complaints come in. The heap size hovers around 40-50mb right after first launching eCW. It seems like there is a correlation that higher the heap size, the slower the application becomes! Refreshing eCW by hitting F5 brings the heap size back down to 40-50mb.

I am wondering if anyone else has noticed the JS heap size?

There was a new feature introduced in Version 11 that pretty much automatically hits the F5 refresh button after the user has navigated through N number of pages (item key Application Refresh Threshold 4033_VAOE). This is great for users who during their session navigate through different pages that count for the set threshold. This works great for the clinical staff because they are going in and out of progress notes all the time and each progress note count as 1 page. Once they reach the refresh threshold, their eCW session refreshes. But our front-office staff generally stays on the Resource Schedule page to make appointments, check in/out patients, collect payments, etc. They may do a lot of actions from within the Resource Schedule but they hardly leave the page, and thus never reach the refresh threshold.

PHM Behavioral Health Module EBO Reports

We are launching the Behavioral Health (BH) module within the Population Health Module (PHM) and will leverage their care plan functionality. I am curious if anyone is leveraging the EBO reports associated with this module to monitor care plan (aka treatment plan) completion and review dates?

BI-Directional Imm Interface

Does anyone have a Bi-Directional Immunization Interface setup and working? We are about 1 step from moving this to PROD but wanted to see if anyone has developed any workflows and/or has a good contact person for them? We have asked several questions and arent really getting the answers and/or are waiting weeks to get responses.

Group Billing issues

We have case managers and BH providers that do alot of group therapy and we have been doing group billing for about 2 years. Twice, we’ve had the group billing function crap out and was fixed after alot of back and forth.  The error message that group billers were getting during those two times, were that they did not have permission to group bill and needed the security setting to modify templates.

Each time we got a technician on the phone they would tell us the users needed the setting: Allow to modify template globally.  ummmm, no!  We only allow our administrators that access and even with it, I could not group bill.  After that, the group billing would somehow be working and we’d carry on.

Now for a third time, the users are getting that error message and I went through the same modify templates globally conversation with a technician and after two weeks of not being able to group bill, we are now told this is a BUG in the system and a hot fix has been requested in a future version of v11.40.xx, which is very non-specific.  Apparently, there is not a workaround and we have no way to bill for group visits (some are 15 people or more in a group) and those notes remained unlocked at this point.  I don’t know the JIRA # and it is not listed anywhere on my.eclinicalworks.com. 

Anyone else having group billing woes or is your group billing is working appropriately?  Maybe there is a workaround we are not aware of.  We are on v11e, current version, eCW hosted.

Circle of Care/Care Team updates

We have CCMR activated and have found a bit of a SNAFU that I am hoping someone can help me with. During the enrollment process of the patient into a Care Plan in CCMR, we select the providers/users that are part of the patient’s Care Team. Once this Care Team is established, it shows up in the Circle of Care area in Demographics (surprise!).
Our issue is we are not able to update a Care Team if the Care Plan has been closed. This doesn’t seem like a big deal until a provider/care manager leaves our practice . . . . and continually shows up on our After Visit Summaries. The workaround we are doing is to uncheck that part of Visit Summaries but we would prefer to remove the former employee from the Care plan . . . .
Support told us that there is no way to do this for closed Care Plans.  Anyone out there have a solution?

Printing Faxing print to Pdf of Medical Record after V11

Since upgrading to V11 ,  we have not been able to Print to Pdf from Windows 10 computers. We use the .exe version.  ECW switched us to the Plugin, but then there was no logo or signatures.  I had them switch it back and we continued to do it on the Windows 7 computers until hopefully they would fix it.  But IT is in the process of replacing the Windows 7.  So now we are having multiple issues and we are now having issues even printing. 

How are others handling this?

Why is “Self Pay” showing up as insurance on green sticky note on patient information header?

We just had our front desk people leave and two new employees start. We also just upgraded to V11. I’m not sure which of those two events caused this, but now the green sticky note (or rectangle) on the top of the progress note and telephone encounter views reads “Ins: Self Pay” instead of showing the actual insurance. The patient has insurances under “Pt Info” and this happens regardless of whether any are marked as primary or not, or whether they are verified.

It was helpful to be able to see the insurance without going into Pt Info. Let me know if you have any idea how to fix this, and spell it out for me if you can (since I am not familiar with the front end side of things). See attached to see what I’m talking about. Thanks!

Alcohol Screening report

I am becoming very frustrating in the reporting that eCW offers.  After the v11 upgrade and the fields they changed, I had to modify many of my eBO reports.  I am all about creating a report.  But when the basic reporting is not provided, I have a problem.  The alcohol screening has been a HEDIS measures for a couple of years, https://www.ncqa.org/hedis/reports-and-research/hedis-measure-unhealthy-alcohol-use-screening-and-follow-up/.  He used to be in the PQRS dashboard.  We are a FQHC with behavorial health. I have placed a ticket with eCW and sent an email to our SAM.  The HEDIS dashboard does not show the alcohol screening.  i am thankful eCW does not the audit C.  I do not understand how a smart form becomes a standard for the system.  i heard other practices say, we paid for the so and o form.  Seems these are forms everyone is asking.  eCW has in a a spot were they are the only ones that can create the smart form and will not make others a part of the basic package.  I am sorry for ranting but this is making my job as implementing and training eCW very difficult.  Our company is beginning to evaluate other medical software’s.  How is everyone else feeling with the current culture of eCW?

EVA…does it work

When we upgraded to V11 we did not turn on EVA.  I’m wondering what everyone else’s experience is with EVA…does it help providers? Are there internet security concerns?  Does your staff feel it’s beneficial?

HEDIS dashboard reviews

Hi all,
I am always willing to invest in something that can improve the overall quality of care and close care gaps for my patients.  What is anyones feedback on easy and accuracy of the HEDIS dash board before I commit to a 3 year term on enabling it.  I don’t get any reimbursement for doing this from our local insurers so it’s purely for patient care.  However the MIPS dashboard is horrible and does a poor job of identifying correct care gaps.  Is the HEDIS much better?  Remember it’s a 3 year $79 per provider per month fixed investment.  If it works well, I’ll do it.

Many folks who are using V11 have mentioned a new look and feel to the e jellybean/quick launch button.  Would anyone on the .exe version of V11 be willing to upload or email screen shots? [with PHI covered, of course] I would like to give my providers a heads up to reduce the irritation factor smile
