eCW Interface Question

Hi All-

We are in the final stages of our EHR vendor selection. eCW is a top contender for my practices, and number 1 from an IT perspective.

I need some help figuring out an interface solution. In short, we need to interface with a CRM such as Microsoft Dynamics. Within Dynamic, we would field all new patient intake, schedule for specific appointment types (consultations), and manage ‘leads’ separate from our patients within eCW. Leads would eventually convert to patients and sent to eCW.

With eCW having a locked-down database, and is limited to HL7, I need help figuring out how we can access, send and receive data between systems in real time. Most importantly, the scheduler needs to work in real time.

Has anyone built anything similar to my interface requirement? I am thinking that we may need to leverage an Enterprise Bus to accomplish this, and possibly hook into Cognos.

Lab Result Font

When a lab result is published to a progress note, the font is a different style and size.  Anyone found a way to make it the same font? 

We’re on V11 web based only.

P2P Facility Registration

Anyone out there utilizing the Facility Registration feature of P2P? We are considering it for one of our clinics in order to receive CCD/CCR documents.

The setup shows a limitation of being able to select only one ‘Authorized Staff’ person that can access referrals/patient records/etc on that facility’s behalf (seems silly considering the same set up at the provider level allows up to 40 ‘Authorized Staff’ users). See attached

If you are using this functionality, how do you get around this limitation (or not)?

eCliniForms:
1. Are .TIFF and .JPEG only formats accepted? .PDF and .docx definitely don’t work

2. What’s the best way to change a .PDF to .TIFF? There is a “Universal Document Converter” online, but we aren’t allowed to add this app to our computers. OneNote and Microsoft Word do not generate .TIFF’s


I’ve looked here/Internet/YouTube for answers to this issues, and I cannot find specific answers.

Tissue Pathology

Our dermatologist does his own tissue pathology and is looking for a better way to document these.  He is currently using a software solution that our lab and pathologist use but it isn’t working well for him.  He wants something that integrates with eCW and is simple to use.

Do any other Dermatologists out there have a recommended solution?

Hi!  Now, we leave our pathology results “open” until the patient has their excision(s).  The problem is that sometimes the patient will be scheduled a few months out for Mohs surgery or has more than one cancer so has more than one excision scheduled which lengthens the time til sign off.  It is clogging up my assistant’s jellybean.  Is there a way to create a running list that would take them out of my assistant’s jellybean?  Thanks so much!!!

We’re currently using Dentrix but are seriously considering using eCW Dental to try and consolidate all our software. Does anyone here use it? What are your thoughts or recommendations?

Will we be able to import all our dental records from Dentrix into eCW? And is this included in the free importing of preexisting EHR data into ECW that they promise? 

Any insights would be greatly appreciated, thank you!

CCM Care Plan Personalized Instructions

Does anyone know how to personalize a Care Plan so it will show up on the Action Plan or Care Plan?
I find myself needing to write individualized instructions like “Your Peak Flow Range is 265-381”
You can add a note that only you can see, but not one that the patient can see.

Medication List

Is there a way in Current Medications List that duplicates of these medications or generic/brand can be grouped together or alphabetized?  Patients will long medications lists have duplicates and this could be a patient safety issue per one provider in my practice.  I know in the right chart panel, you can group by date or medication.  Any solutions to this?

Girish - HUGE patient safety issue with Medical History edits not saving

We are running the executable (V10 SP2.5.48). There is a huge patient safety issue where changes/additions to the Medical History are not saved. This happens in at least two extremely common and reproducible instances:
1) When you click on an existing line of the patient history to edit, edit the text, and then click on the white space below the last medical history line (see picture) to exit editing the line, the changes you made to the line show up on the Medical History popup, but do not save when you go back to the Progress Note.
2) When you click “Add” and add a line of Medical History, and then click on the blank space below the last medical history line you just added (see picture) to exit editing, the line you just added shows up on the Medical History popup, but is not saved when you go back to the progress note.

Example:
- Open a patient’s progress note
- Click “Medical History”
- Click “Add” to add a new condition or edit an existing line
- Type some important medical condition that you want saved to keep your patient alive “Severe unstable angina”
- Click anywhere in the white space below the the last medical history line (see picture) to finish your addition/edit
- Click the red X in the upper right to go back to the Progress Note
- Watch as nothing happens and your medical history change is lost

This is probably the scariest thing I’ve seen with eCW. I’m pretty sure I’ve noticed the effects of this for years, because I’ve had assistants tell me that they added something to the medical history or changed a line, but it wasn’t there when I reviewed it. However, I was only able to figure out how to reproduce it recently. How much crucial information has been lost, and how much patient harm has this caused?

Having a computer science degree myself before my dental degree, I’d consider bugs like this indefensible. Clicking away from the cell you are editing in a spreadsheet program to save the changes you just made is common user behavior (this is my behavior almost 100% of the time to make sure my change got saved). The white area below the medical conditions is one of the most logical places to click to prevent you from editing another line. The fact that the change registers in the popup also indicates to the user that the it was acknowledged. To have it then fail to save on the progress note is unbelievable.

My other concern regarding safety issues in general is the way in which they are tracked and processed. I tried to search for this issue on my.eclinicalworks.com, but the list of issues unnecessarily truncates the majority of the title of the issue (e.g. “Patient Safety Advisory: Browser Only - Global Alert Notes Information… NEW”) while leaving empty space to the right of “NEW” where it could have finished the title. It doesn’t even display the full title when you hover over it. Even more incredibly, the search function doesn’t actually search the full title or body of the issue, it only searches the truncated title! Finally, to have significant issues sit around on that list for months with the proposed solution “Don’t trigger the bug” and the status “We’re working diligently on it” is very concerning.

I’m posting this here for a few reasons:
- To see if others can confirm what other versions have the same issue
- To make others aware of the issue
- I don’t have privileges to initiate cases from my.eclinicalworks.com
- When I report something over the phone, I’m either told that it is expected behavior or a known issue that is fixed in the browser version
- Our SAM usually monitors the forum
- I’d like to know (preferably from Girish) how things like this happen

Am I overreacting? Isn’t there some significant oversight on eCW due to the settlement and previous safety issues? Obviously the handling of current patient safety issues isn’t working, so what can we do? The integrity of medical records, particular medical histories, should be the number one priority of an EHR. If my PCP used eCW, I would absolutely find a new office, no matter how good he/she was.

“Package Not Enabled” in Right Chart Panel

In version 11, does everyone’s Overview tab of the Right Chart Panel look like this- with HCC RAF Scores- Package Not Enabled and HCC Gap Recommendations- Package Not Enabled appearing ABOVE Current Medications and Allergies (see attached)?  If so, seems like a marketing technique with implications for patient safety, putting relevant info on medications and allergies underneath an alert that we having paid for a feature.  At the very least it could be collapsed with no info in it, just the heading.

In-Place Upgrade Windows Server 2012 R2 to Server 2016

Everyone,

I was hoping someone might have some experience with this.

I have performed many in-place upgrades of Windows Server and I rarely have any issues.
It is nice to not have to migrate and deploy all new software, especially when already virtualized.

However, I have never done an in-place upgrade of a Windows server running eCW.

Can we do this with a server running eCW?

If so, I could see this being huge for saving time and money.

Please let me know your thoughts.

Limiting CPT Codes per Insurance

We have a case where we need to only allow a handful of CPT codes to be billed to a particular program.  The program reimburses for those codes only.  We’ve built the program as an insurance because we send them the 1500 form for claims.

I am trying to figure out a way to prevent any other codes from being put on the progress note.  I thought I could use the Valid Insurance/CPT/ICD Combinations to set this up.  I have set up my rule, created an encounter on a test patient that should trigger the rule and clicked done.  No alert appears to warn the provider that there are problems.  I’ve read several posts on here that sound like it did do this.  I’m guessing that I am missing something.

I have selected the insurance company in the “select insurance” section.
I have added the CPT Codes that are valid in the “cpt codes” section.
I have added the ICD Codes that are most often used on these encounters in the “icd codes” section. 

I cannot find any documentation in ecw’s knowledge base to verify that I am setting this up properly.  I know sometimes my logical mind doesn’t process the same as ecw.

I tried using the Invalid Insurance/CPT/ICD Combinations for one of the CPTs that I don’t want to check my logic.  That alert appears as expected when clicking the done button on the progress note.  This would work except there are only 14 cpts that I do want so trying to eliminate all the others would be a maintenance nightmare.

Anyone have an issue with Locked Accounts today?

We came in today with 30+ accounts locked, one of our guys reached out to eCW and they said they had implemented a new security policy over the weekend apparently where any account that had not logged in for over 90 days will now be locked. Most of these users were inactive users, .

We checked back after lunch and now only about a dozen remain, and these all seem like active users. So not sure if eCW went through another update to only lock active users who hadn’t logged in for 90+ days? I didn’t read anything about this anywhere and was wondering if anyone experienced anything similar today?


V11 “Appointment already exists” warning

I thought I already saw on a post on this (perhaps even made it myself) but I cannot locate it. Are other people getting the attached warning whenever you attempt to modify an EXISTING appointment? The system appears to not be looking to see if the appointment that is already scheduled is the same appointment you are in currently. Any work around?

v11e buttons randomly grey out

We have gone to v11e and experiencing several issues.. One is randomly the medical history , lab labels, and select scanner button are greyed out.. We clear browsing hx in chrome and clear client side cache to get the buttons back.. However it happens multiple times a day.. Has anyone had the same experience? Any suggestions? ECW has not been too helpful..

Anyone using the PDMP product in V11 yet? (Appriss Narxcare controlled substance report query)

Curious to see if it allows RN’s/MA’s to run the query while rooming the patient.

Also wondering if logging is working as expected.

We’re currently using Dentrix but are seriously considering using eCW Dental to try and consolidate all our software. Does anyone here use it? What are your thoughts or recommendations?
