How often do you do PHQ2/9

Apparently for MIPS, every eligible encounter requires a PHQ2/9. If it’s not done, patient is non-compliant. We know of another FQHC ecw facility who only does it once a year on the kiosks so I’m questioning everything. I’ve found the MIPS document on the portal and it clearly states it’s every eligible encounter. I’ve also read documents about artificial intelligence in ecw so I take everything from them with a grain of salt.
