Orthopedic Forms - KOOS, HOOS, & Promise 10

Hello,

We are in need of reporting scores for Orthopedic Forms: KOOS, HOOS and the Promise 10. We were hoping that eCW had these created, however they do not and they are still in a pause for taking custom requests. Has anyone else found a solution for forms like these that need a score populated and getting the result in eCW? My only thought right now is to use a form from the web and then adding in a spot on a template for providers to enter the score.
