We are just getting ready to start building our Care Planning module in eCW. We are not going to utilize CCM, just the module that is formerly known as CCMR. Any advice? Anyone want to share the templates they’ve built? Our first focus is diabetic patients, then we will be looking at Hypertension, Obesity, etc…
