Billing Claims and Encounters

Hi all,

I have noticed that when a billing claim is created for an encounter, it then becomes independent of any billing codes or ICD-10 diagnoses entered into a patient note. If there are any changes or if the claim was created by our billing before a note was completed, we have been having to update ICDs and CPTs on the note and then do the same thing again on the claim.

This seems redundant. Is there a way to “refresh” a billing claim so it pulls forward new info from the associated encounter/note so we don’t have to do it all twice manually? Or some other solution to this problem?

POS code HELP

We are trying to create and file tickets through a telephone encounter and if you manually change the POS from our normal filing code 11 to a 02 as required it will reject the claim stating it does not match our facility POS code 11. We can go over to the claims screen and manually open a claim but then Medicare won’t have the encounter to view.
Does anyone know if this is being waived by Medicare or will we have to swap over our Facility ID everytime we file from schedule to televist?

Automatically confirm appointments made through Healow?

Any one know how to do this so I don’t have to wait to call them on monday? Basically, when a patient makes an appt on the healow app it goes to my schedule but the visit is coded as 45 minutes and it only blocks off 15 minutes on my schedule and makes me go in and confirm the appt and add the patient, whats the way to get this done automaticall?

unexpected update problems- CVS and CCM

Anyone get a surprise yesterday on the SAAS .exe version.  CCM is no longer in the top of the overview column.  I thiough it disappeared but support told me it’s moved to a much lower location on the right tab which is ridiculous.  CCM is hard enough for staff to remember to document time spent, moving it to a unviewable place means let capture of time and hence lost revenue.  I also noticed e-refill scheduled scripts don’t have a X automatically in the refill needed.  I also noticed a few more errors in schedule drugs being transmitted.  I spoke to ECW and they say a fix for all the above is in the works but several weeks away.

What’s going on here.

Patient E-mail list

Is there any way to get a patient e-mail list so we can send an email to all of our patients through constant contact? It would be great if we could get a smart list (i.e. active patient’s we have seen over the last 3 yrs)

new CCM code for providers

CCM now pays 99491 $84 for 30 minutes spent by Md or NP outside of a physical doing any CCM stuff.  I have the CCM module but I don’t see how it differentiates times spent by a provider entering CCM time versus my RN or MA.  Is anyone aware of an update that allows for proper documentation of 99491 code.

Televisit volume on Provider side

We are on 11 exe and we are experiencing random pc’s that we can’t hear the patient.  However, if you use another application the volume is ok.  We are trying everything.

Currently we need to add an external pair of speakers in order to hear the patient.  Not acceptable at all.

Just wondering if anyone else is experiencing this random issue.

New telephone encounter Reason not showing up in drop down

Noob question here. I’m kind of embarrassed! Last week I added a new “Reason” under Admin > Notes > Tel Enc Reasons. And naturally it doesn’t show up in the drop down list when we you open a new telephone encounter. I went back into Admin and confirmed that it’s there and - sure enough - it’s sitting pretty right in the middle of every other Reason in the list. But it doesn’t show up in a new telephone encounter. I’m about to pull my hair out. Am I missing something? Thanks!


TeleVisit Visit Types

Are you creating and naming your TeleVisit visit types = “TeleVisit” or something similar, and checking the box to create it as a Healow TeleVisit? 
We have so many specialties and visit types (Gender clinic, PrEP, Behavioral Health, so many more plus the usual NP and EST appts, wondering how you are distinguishing between them as TeleVisits.  Any info is appreciated.

Telemedicine Alternative Services

We are looking for an alternative telemedicine service.  We are using eCW’s Healow solution, but since we can’t get iPhones to work we are looking at about a 25-30% success rate.  We need a back up plan.

We’ve looked at Doxy.me but the free version looks cumbersome (no text or email).  We are discussing the Skype and google options as stop-gap measures for patients that can’t connect via eCW.

What is everyone else using and how well do you think it works?

So I’m now upgraded to v11, and after spending a bunch of money, really not happy with the upgrade.

so far the newest thing is that i can’t put dispense: 3 months supply.  we do it all of the time with birth control pills or whatever and now apparently surescripts won’t let us that, of course I said what if i just want to print a prescription and hand it to the patient and it also won’t do that.

very frustrating.

so after upgrading to a much better server and lots of dollars spent, so far no real improvement in my workflow.

TeleVisits - Web Enabling Patients & Emails Not Received

Hello,

We have started using TeleVisits successfully with multiple practices, however, with one practice on a separate database we are experiencing issues with getting the initial web enrollment email out to them. We have the Admin setting turned on to send emails to newly enabled patients. But we are seeing a large portion of our “Access Credentials” email in the Admin Email Fail Log. This is happening for both the web enable set up email and for password resets. Is anyone else having a similar issues with a large number in their failed log?

No Healow App Dependency on Phone - TeleVisits

This morning eCW released an update that allows users to access televisits through their phone without having to download the healow app through the email reminder link. We have been able to successfully test this out using iphone but have little to no success with android products. I wanted to hear if anyone here had any better experience.

Flash Player No Longer Supported

We have seen several tech articles on this, and also there is now a message on the top of Chrome at launch that states ‘Flash Player no longer supported after December 2020’.  I know this is not coronavirus related or high priority, but does anybody have an alternative since V11e uses the flash player to take pictures?  We tried Edge Chromium with V11e and it also uses flash player.

Inactivating Furloughed Providers

Due to the COVID-19 crisis, our medical practice furloughed over 300 employees including 53 advance practitioners. Their roles include NPs, PAs, CRNAs, Physical Therapists, and Audiologists.  Has anyone ever encountered this at their practice?  We are struggling with labs and imaging resulting into the buckets of the NPs and PAs that are no longer working.  The furlough could last anywhere from 6-12 weeks.

I am trying to obtain some info from eCW regarding their eCW licenses.  I am unsure if we can deactivate their eCW license.  If we do that as well as their Surescripts license, we will likely have to re-enroll all with their EPCS upon return.  This would likely include them all needing new FOBs for EPCS which is not feasible from a cost perspective.

Any input would be greatly appreciated!


TeleVisits - blurring the background for provider

Does anyone know if there is a way to blur the background for the providers while doing an eCW TeleVisit?  While we’ve asked providers to place themselves more in a corner or such, but some are still concerned about personal space being seen by the patients.

Televisit Consents

For those of you have not done Telehealth before (like us) and are now thrown into during this emergency.  What are you doing for consent in the Progress Note. 

The consent recommended by legal, is a 2 page document.  That will work great to have the patient sign after this crisis moving forward.

I don’t want to type this whole thing and have the provider read it to the patient and have it have to populate the note.

Is there any short, sweet, highlight, to the point information that is most important for the provider to read and gain verbal consent from the patient.

Televisit for patients that don’t have an email address

What’s everyone doing for televisits on patients that don’t have an email address? We have a patient on the schedule today and we’re scrambling to find a way to connect with him.

Wondering if anyone is using televisits through eclinicalworks. Our biggest problem has been connectivity issues and errors that patients get on their side (no appointment showing up for them etc). 100% of the time we cannot get through a televisit without loosing connection 2-3 times.
We have the fastest business comcast speed available in our area.
Many times we start the televisit and then have to finish the televisit on spruce (which has perfect connection). Plus we are paying $2 per visit with ecw televisits and if we use our other apps we pay nothing extra. We would rather use ecw as patients don’t have to download an extra app. We are about to change to att fiber and we will see how it goes.

So I am wondering if it is even possible to have a good connection with eclinicalworks? What is everyone’s experience?

eClinicalTouch requiring a iPad PIN

We updated one of our iPads to the latest version of eClinicalTouch (3.5.2) and now it requires a PIN to be setup. Without a PIN, it does not let you log into the app. I reviewed the release notes, but obviously there was no documentation of this change. Has anyone else updated their eClinicalTouch?
