Our practice uses medical assistants and RNs to complete certain sections of the chart, and Medicare guidance specifically allows ancillary staff to enter information derived from the patient for the ROS and/or PFSH. However, the physician must provide a notation in the medical record supplementing or confirming the information recorded by others to document that the physician reviewed the information.
Is there any way to add a statement such as: “I have reviewed the documentation in this chart in its entirety and confirmed its accuracy,” at the bottom of the chart in such a way that it automatically appears with the electronic signature whenever you lock/sign a chart?
