Document under Assessment Plan section for my notes?  What’s better

Hi,
I have always put my details of a problem under the Assessments category:  eg: CAD - doing well, Echo 9/2017 Ef 55%,  last stress test 2013 normal.  cont statin/bb/asa.

I do that because if you fax the medical summary to the hospital, it includes that section of notes which is useful info for the hospitalists caring for my patients.

But I see a lot of ECW providers now doing that personal documenting under Treatment Care plan box.  That does not transfer in the medical summary unfortunately. 

What do most people prefer?
