How to give patient’s a risk score

Hi Everyone,

We are trying to find a way to give out patient’s a risk score in eCW and then be able to pull reports based on the information.  Is anyone else doing this?  We were wondering if the Population Health feature that eCW has would work for this or if we need to just create our own risk score like a code and then have the provider add it when they are charting so we can then pull a report based on the risk score.  If anyone has any ideas how to do this or is currently doing this we would appreciate any information.
