MIPS data for out of office clinics - How do you capture this data?  General surgery practice

We are a surgical office where only 20% of our patient load is seen within the office and the doctors physically dictate within the progress notes.

The other 80% consists of surgeries and hospital based clinics.  All of the notes/dictation is done in the hospital EMR, not ECW.  We scan this information into patient documents.  We use out of office encounters function to track the encounters on these and create the claims.  However, I am running into issues on how to capture the MIPS data for those patients.

For example - patient seen in a clinic out of office, we bill charge 99203 and want to capture the RX verified measure.  From what I understand, I can only get ECW to capture this if I have an appt on the resource schedule which prompts a progress note to be accessible.  From there, I can do the RX verified.  If I use the Out of Office Encounter option, I don’t have the capability.  I think my only option at that point would be to add the MIPS code to the claim itself and then possibly run a report to get my entire list for reporting.

This seems extremely cumbersome and overwhelming as I have to pick multiple new measures this year due to deletions and specialties leaving our group.  Is there anyone out there who has a similar work flow or are we the exception to the rule?
