Documenting medications (oral, injections, and nebulized) administered during an office visit

We have a pediatric practice in which we administer a number of medications in house, Tylenol, Motrin, Prelone, dexamethasone, atarax, zyrtec rocephin, and nebulized medications such as albuterol.  In the past we have documented the dose and the lot number on our paper progress note.  How are other practices ordering and tracking whether such medications have been administered?  For example, if a child comes in with a 104 fever how does a practice document an order for Motrin 100 mg (1 tsp.) and that the order for Motrin has been executed.  In some posts I have seen that other users have documented such medication orders in the immunization field, if this is the case does this medication appear on their immunization record?
