We are working on trying to create a better workflow for our telephonic case managment calls. We have never successfully used the Care Planning module under PHM. Is there anyone successfully using this that would like to share their workflow? We would like to register a group of patients, assign an outreach clinical call to multiple different staff members, have the staff members document uniformly in a template (in the PHM module?), and have a reminder pop up every so often (week/month) to perform another outreach. When the outreach is completed, the reminder would be suppressed.
Is anyone doing that currently and want to share some tips? I can’t even figure out how to smoothly access the PHM care plan itself from the care planning module (unless I go out to pt hub->PMH hub->create virtual visit.) Is there a quicker way?
