Progress notes left open by providers no longer at the practice

I’m curious to know how other practices handle progress notes that are left open by providers who are no longer working for the organization. I know the best thing to do is to have the provider lock all open charts before being separated but that doesn’t always happen.

Do you keep the notes open or do you have someone, like a supervising provider, go and lock the charts? If a supervisor locks the chart, is an addendum used to indicate that the person locking the note is not the author of the note?
