Telephone Encounters - Default Facility

After V11 update we have a user that when she creates a telephone encounter ‘Facility’ is blank. It’s not defaulting to the patient’s facility. I’ve looked in ‘My Settings’ and don’t see anything. It’s only happening to this user.
