Generating letters

I would like to be able to generate signed letters to patients.  My IT staff has presented me with a Word template which contains pretty much everything I need with the following exceptions:

1.  The date of the letter.  This should be the actual date I am generating the letter, not the date on which it is opened (MS Word does not seem to have a field for the date a document was first created, which means that if you open a letter later you get the current date instead of the date it was written).

2.  My signature: I know that little picture is in here somewhere.  It shows up in my prescriptions when they are faxed, on my imaging and lab orders, etc.  It should be available as a picture field that can be brought into the Word document (so I don’t have to print it out, sign it, fax it, scan it back in with the actual signature on it).

3.  Office address, phone/fax numbers, email address, URL, etc.  We are a large multi-site practice and this needs to come from the site information, preferably the site at which the patient is seen by that provider (and not the site at which the provider happens to be on the day s/he is typing the letter, for those who move around between offices).
