Just thought I would post this solution in case anyone else was having this issue. Recently we noticed that our medical history and surgical history sections were showing in a run-on paragraph instead of a bulleted column in an unlocked progress note, but once locked, the histories are back to a bulleted column. This of course is a safety issue as it makes it incredibly hard to read and provides an opportunity for something to be missed. An incredible tech at eCW, Rajkumar brought me right to the fix. Somehow, a default setting was changed for all our providers. Just go to: Left Navigation band > Menu > File > Settings > My Settings > Defaults > Default style in Progress Notes change to bulleted. Log out and log back in, and everything was back as it should be. Possibly this got changed with a recent patch, but the fix was easy.
