Unstructured Allergies Initiative

We are still on V11.40. I see there are enhancements in the newer versions but do those enhancements help resolve historical data? Is everyone correcting these unstructured allergies on each patient as you go? How can I tell if an allergy that is listed in the progress note is structured? When I ran the EBO report, the number of allergies listed made me nervous about the scope of a project to correct them.

A significant fraction of medication errors and preventable adverse drug events are related to drug-allergy interactions (DAIs). Computerized prescribing can help prevent DAIs, but an accurate record of the patient’s allergies is required. 1

Why Should You Be Concerned About Unstructured Allergies?
Structured allergies, also referred to as coded allergies, are allergies documented in your eCW database by selecting the allergy name from the medication compendium. By contrast, unstructured allergies, also referred to as uncoded allergies, are allergies documented as free text without selecting the allergy from the medication compendium (e.g., typing “penicillin” rather than selecting it from a compendium menu). Importantly, unstructured allergies will not trigger drug allergy alerts in the eCW software. As a result, users may not receive appropriate alerts, which may lead to the prescribing of a medication to which a patient is allergic or may lead to other adverse consequences.

Across its customer base, eCW has identified instances of patients being prescribed medications to which they have a documented allergy, relating, at least in part, to the unstructured documentation of the patient’s allergy. Through our Unstructured Allergies Initiative, eCW will help customers reduce the use of unstructured allergies in their eCW database, free of charge.

To help you review and understand the scope of unstructured allergies existing in your patient charts please review your Safety and Compliance Dashboard

eCW is committed to helping you mitigate the potential risk caused by unstructured allergies in your patient charts. At no additional charge, eCW is offering you the opportunity to receive the following:

Identification of unstructured allergies in your patient’s charts that can be clearly and readily converted to structured allergies (i.e., -converted to an allergy in your medication compendium)
Application of a mapping tool to automatically convert the identified unstructured allergies to structured allergies, following your approval
Delivery of an exception report, listing any unstructured allergies in your patient’s charts that could not be clearly and readily identified for conversion to structured allergies by eCW
Technical support from eCW to assist you with understanding how to implement the exception report so you can, to the extent feasible, convert any remaining unstructured allergies on the exception report to structured allergies.Please log a support case under Open a case
How Do I Participate?
No action is required by you at this time to participate in the Unstructured Allergies Initiative. eCW clinicians are reviewing and identifying unstructured allergies across our customer base that can be clearly and readily converted to structured allergies. You will have an opportunity to review this list, and eCW will not make changes to your database unless you approve and choose to execute the conversions.

Until these functionalities are made available, from the Progress Note add the coded allergy from the allergy list and then remove/delete the uncoded allergy. Metrics provided in the Safety and Compliance Dashboard will provide you with details of current patient charts where uncoded Allergies exist. We anticipate additional functionalities to be rolled out before the end of the first quarter of this year (2021) and any update regarding the same will be provided on my.eclinicalworks.com.
