Electronic Prior Authorization Through Surescript?

It appears that Surescript has offered this for a while. Is anybody using that, inside or outside eClinicalWorks? If not available, is eCW planning to use that? If not, why not? This might be an upgrade that would dramatically improve workflow and functionality. Any input?

As the surescript website is putting it correctly:

“Prior authorization is the biggest headache facing physicians today: ninety-one percent describe prior authorization as primitive, time-consuming, expensive, and frustrating”

Reconciling labs for providers that have left and are inactivated

So, I’ve been working in eCW for 8 years and reconciling labs for almost that long and this is new one for me.  I have labs that came back unreconciled for a provider that has left and is inactivated in our database.  Not sure I’ve had this issue before, but the group I am with now, seems to have a lot of unreconciled labs due to several factors, mostly not enough auditing (or caring) at the front desk to get the patient’s demographics exact…don’t get me started….
Anyway, how are you handling the labs that don’t reconcile that were ordered by a off-boarded provider?  Do you create a new order and assign to a provider that may be covering those?

Copy to Medical Hx

When in the Problem List, you are able to select a problem and “Copy to Medical Hx.” 

The diagnosis or “name” is then copied to the Medical History. However, none of the other fields like “Specify” and “Notes” are transferred to the Medical History. Is the the way it’s supposed to function? Or is there a setting to allow it to copy the other fields.

Implants

Anyone have Implants enabled, do you have any best practice or other usage workflows, that screen is massive and we want to be sure to fill it out correctly.

We thought we would be able to search the patient in the registry as well, but that doesnt appear to be the case in a quick look. Can you not verify implants for a patient this way?

NDC Codes dropping on Claims for Imm/injection Admin codes

We are utilizing the imm/injection Lot Number setup area to reflect the appropriate NDC code for that drug.  The issue we are running into is that if we have an imm/inj set up with an Admin code, the NDC that is built on the lot number for that drug, also drops under the admin cpt on the claim.

We’ve asked eCW and they were able to find an item key that prohibits the NDC code from dropping for admin fees IF it’s set up on the CPT itself, but says there is no item key or setting that prohibits it from dropping on the Admin fee if the NDC is setup at the Lot Number level.

Has anyone else run across this and if so, any ideas?  Right now, our billers are having to manually remove these NDCs from the Admin CPTs before sending the claim out the door.

Any input is greatly appreciated!

ecw support- how to use them efficiently

I wonder if there is an easier way to handle tickets with ecw. Right now we open a ticket and support calls us during clinic hours and we cannot drop everything and work on tickets with them. We have requested that they call on certain days or times (like please call Tuesday morning between 8am and 12pm CST) which are our admin days or lighter clinic days, but they never do and end up calling at the wrong times. We play phone tag. The tickets get dragged out for weeks. Today they randomly called during clinic to install fax server software. We told them to call back tomorrow because we had faxes going out and we cannot stop them for an install in the middle of clinic. I bet they won’t call back tomorrow.


I wish eCW had some type of scheduling platform for non urgent tickets. We could submit three dates and times for them to try calling us to work on tickets and they can reach us without difficulty and get the ticket taken care of.

training tips for going to 11e from 11exe

We are planning to gradually transition to 11e over the next couple months.  What are your best tips for training?

I’ve been using 11e for several clinical sessions over the past few weeks and have pages of notes and questions on where to find things, how to do them differently. 

I’ve watched some of the eCW University videos (based on 10e, created 3 years ago I think?) and they are helpful for orienting to the new locations of some things, but not all.

How did your practices approach training current 11exe users on 11e?

OverDose Drug Warning

All,

Has/Does anyone know if there is an item key, setting, security option for a drug overdose warning to “pop up” when prescribing an rx.

I.E. Losartan 300mg daily. (only comes in 25, 50, 100)

When ordering/prescribing this med, there is no warning about an overdose.


In need of security setting to copy appointments

First off, I really dislike browser application!  That being said, we are having a tough time getting all users in the same role to be able to right click on the appt and click on COPY an appt.  Not all users in the same role are seeing the COPY option.  Any ideas?  Ive reset their profile settings a couple times…still an issue

EPCS Mandate? for all providers

All,

Has anybody here heard, or started working on getting all there providers setup on EPCS? MD’s, PA’s etc…Doctors and mid levels.

(I am not a doctor by any means) but this was brought to my attention by our clinic director thru some PA’s that when to a recent conference.

Users unable to open Vaccine hyperlinks in progress notes

Our nurses are all in the same role, but at least three of them are unable to open vaccine hyperlinks in the progress note to be able to complete the vaccine details window.  We thought it had to do with the plug in (we’re on 11e) but uninstalling and reinstalling that did not help.  We had two of them log into another computer of a user that is not having the same issue and logging into the computer under their own credentials, still does not allow them to open the vaccine hyperlinks.  Any thoughts how to resolve this?

Care Planning

We are working on trying to create a better workflow for our telephonic case managment calls.  We have never successfully used the Care Planning module under PHM.  Is there anyone successfully using this that would like to share their workflow?  We would like to register a group of patients, assign an outreach clinical call to multiple different staff members, have the staff members document uniformly in a template (in the PHM module?), and have a reminder pop up every so often (week/month) to perform another outreach.  When the outreach is completed, the reminder would be suppressed. 

Is anyone doing that currently and want to share some tips?  I can’t even figure out how to smoothly access the PHM care plan itself from the care planning module (unless I go out to pt hub->PMH hub->create virtual visit.)  Is there a quicker way?

Visit Summary Not Printing Correctly with “Send” Button

Strange problem for many years

If I use “Send” function from progress note to print “Visit Summary”, about 1/3 of the time, it will not print correctly-letters are dropped from the text and spacing between words is lost intermittently in text.

This occurs with hardwired and network printer. Printers are typical HP printers.

If I print again, it will print correctly.

IT cannot figure this out (in their defense, always hard to figure out intermittent problems). They blame the printers, but I don’t agree-every other program prints fine.

Anyone else seen this?

Change date header on surg history from mo/yr to mm/dd/yy

Does anyone know if this is possible? You can input the full date, but because the header says mo/yr most of the staff are just putting that, and usually the pts know the exact date. Is there any way we can change the header to mm/dd/yy instead?

Clinical Rules Engine not working

Is anyone else having an issue where the CRE is upcoding the PN by dropping an extra ICD into the claim and note? In our case E11.65 is dropping. I don’t have it enabled anywhere in the CRE and eCW has been able to duplicate the issue, but not resolve it. I’m almost tempted to check each provider’s templates to see if there’s an errant code in there, but it just doesn’t make any sense.

This really sucks because we’re missing out on incentives because our staff won’t consistently add CAT II codes during a visit.

Since V11 getting error when sending Narcotics

Since we switched to V11,  intermittently when we send a Narcotic we get the following message.

“An error has occurred, hence you cannot send this Controlled Prescription online. Doctors are required to give a fax, handwritten or printed prescription with doctor’s signature” .

They exit and try again and it will go through , sometimes they have to wait a few minutes and resend and it will go through. 

Has anyone had this issue?

billing rule

If a patient is in hospice,  how can the billing /claims team be aware to append the hospice modifier rather than the provider free-texting in the billing box, pt is hospice. There must be some setting in ECW that makes a patient’s insurance -hospice an automatically requests hospice modifier.

eCW Version 11 - 2015 version PHQ2 Form

It seems like the new PHQ2 (2015 version) now has a score associated with it.  Still two questions, each question now has a point value of 1. The new 2015 PHQ2 form now only initiates the PHQ9 form automatically if the score is 2 or greater??  The old version initiated the PHQ9 form if EITHER question on the PHQ2 form was answered as yes.  Now both questions have to be yes in order for the 9 form to automatically open.  The UDS measure didn’t change - it still states if either question on the PHQ2 form is yes, a follow up must be done (which the PHQ9 form satisfies that).  How is everyone else on version 11 of eCW dealing with this??  Our depression measure has dropped, partly because the PHQ9 form now doesn’t open if only 1 question on the PHQ 2 form is positive.

Rearrange order of items in ROS category?

How can I move some items down a little or rearrange the order of items in an ROS category?? There’s got to be a way to do this, and I must be missing it.
